


Crossroads Home Care and Hospice
is dedicated to the care of the sick in their homes. We believe that the sick
are entitled to the highest quality of personal care. We believe that the
elderly have special needs and that these needs should be evaluated and met in
the most sensitive manner. We also believe that the family of the sick are also
in need of support and instruction. We believe that all persons regardless of
the race, creed, color, age, or sexual preference are entitled to the same high
quality of care. We believe that nursing and health care in the home is a
viable alternative to hospitalization. We believe that Crossroads Home Care and
Hospice has a responsibility to prepare and educate its professional,
contractual, and ancillary staff to coordinate and deliver the most efficient
services available. We believe that the community involvement is important and
we resolve to participate in community efforts for the care of the sick in
their homes. We believe that research and teaching health professionals is
important in improving patient care and we resolve to participate in
educational activities whenever possible. We believe in the total care of the
patient and as a team we have staff that will in every possible way meet the
spiritual, emotional, social, physical, and psychological needs of our
patients.
Crossroads Home Care and Hospice
subscribes to the concept of health defined by the World Health Organization
as:
‘…a state of complete physical,
mental and social well being and not merely absence of disease or infirmity.
The enjoyment of the highest attainable standard of health is one of the
fundamental rights of every human being without distinction of race, religion,
political belief, economic or social condition.’
To this end, Crossroads is
committed to providing a high performance of health care essential to the
improvement of a client’s physical and psychosocial condition. At the same time,
we recognize and respect the broad base of client rights, in all care and
service delivery, and the need to effectively conduct all business and direct
care activities related to this care in an ethical manner.
Crossroads does not discriminate

Furthermore, Crossroads respects
the autonomy and privacy of the client and family to make decisions and choices
affecting their present and future health status.

An adjunct to this concept is the essential role of evaluation of the Agency’s
services and applied research in improving those services.
The Agency Board of Directors has
developed a mission and vision statement based on the above philosophy. All
staff are educated regarding the above and ongoing monitoring will occur to
assure that the Agency’s mission is sustained over time.
