Criteria for Hospice Home Care under Medicare.
- Patients must have Hospice diagnosis of six (6) months or less to live
We provide the following:
- Intermittent visits by nursing personnel, a homemaker,
home health aide, social worker, chaplain, volunteers and other
professionals as defined on the Informed Consent Form.
- Intermittent visits and/or consultation by auxiliary team members providing dietary, physical occupational and speech therapies.
- Extended
hours of care in the home in times of medical crises, as determined by
the Hospice Team and defined by the Medicare Hospice benefit. Hospice
is on call twenty-four (24) hours a day, seven (7) days a week.
- Medical
supplies, equipment and prescription medications (less deductible, if
applicable) that are related to the terminal diagnosis.
- Hospital
care for acute pain and symptom management that cannot be achieved at
home. Short-term inpatient care is provided only as arranged by Hospice.
- Respite care to relieve the family/caregiver for up to five (5) days per stay per period.
