Our History

Crossroads Home Care and Hospice was founded in 1995 with the belief that health care is at its best when the health care providers are members of the community. Our unique brand of compassionate end-of-life care involves family and loved ones, fundamentally improving and changing the way people approach serious illness. As a result, the dialogue around this phase of life has deepened and our understanding of life as a whole is richer. Crossroads Home Care and Hospice provides the finest in hospice and palliative care. At Crossroads we embrace the philosophy of caring inspired by the monks in Ireland and England where the modern Hospice movement began. One of our first Hospice Patients, Esther Oppenheimer, said it best: "Without Hospice Care, I would have had nothing to hold onto these last few months of the rest of my life...
