Criteria for Home Health Care under Medicare:
- Patient must be Home-bound and unable to function outside
of the home without a considerable and taxing effort (i.e. with the aid
of another person and/or device).
- Have an exacerbation of an existing illness of onset of a new disease process
- SN, PT, or SLP need on an intermittent basis
- Administer IM injections
- Teach administration when appropriate
- Supervise administration
The patient must be post hospitalization or referred for home health from the physician's office. Services Available:
Diabetic Teaching
- Signs and symptoms of hyper/hypoglycemia
- Skin and foot care
- Insulin injection technique
- Diet teaching
Wound Care
- Remove sutures, care of drain, packing etc (with specific orders)
- Observing surgical incision
- Teaching aseptic procedure
- IVAC Certified
Eye Care (Post Cataracts)
- Sterling dressing
- Assess interactions of drops & medications with an additional skilled need
Catheter
- Inserting or changing Foley catheter
- Observing contents, amounts etc.
- Observe hydration
- Obtaining sterile urine
- Teaching-catheter care, including irrigation
- Straight cath for residual
Vital Signs
- Assess BP readings, apical/radial pulse quality and rate,
respiratory rate and temperature. If patient is stable, vital signs
alone are not alone skilled care.
NG Tube, J Tubes, G Tubes
- Inserting or changing NG Tube
- Instructing care of tube, feeding formula, etc.
- Instruction on use of pump.
Lab Work
Only in conjunction with an additional skilled need.
Venipunctures to determine blood levels but not necessarily limited to:
- Protime
- Blood glucose
- CBC
- Blood specimens
- Chemistry profile
- Drug levels--diagoxin--theophylline and dilanti
Ostomy Care (case-by-case basis)
- Change of new stoma, dressing, observation of discharge, care of skin
- Teaching ostomy care
- Irrigation
- Diet of colostomy patient
- Skin and wound assessment and care
Bowel & Kidneys
- Digital removal of fecal impactions
- Bowel and bladder training
Orthopedic Care
- Post-up (i.e. TKR, THR) care and observation
- Cast care
- Teaching, rehabilitation measures
IM Medications
